We would like to invite our registered patients to join a patient participation group. Being a member of the group will allow you to share your views as we aim to improve quality and develop services at Brunswick House.
As a member of the group you will be invited to complete periodic surveys from the surgery to help inform our quality service development.
If you would like to join the group please click on the join button above and follow the on-screen instructions.
Please tick the box "Would you to receive e-mails from this site" when prompted, to allow the surgery to contact you.
You will be sent an e-mail when you register, asking you to verify your e-mail address. You will then be able to log in as a site member.
Commonly asked questions about the patient participation group
Q Why are you asking people for their e-mail details?
A We would like to be able to contact people occasionally to ask them questions about the surgery and how well we are doing to identify areas for improvement.
Q Will my doctor see this information?
A This information is purely to contact patients to ask them questions about the surgery, how well we are doing and ensure changes that are being made are patient focused. If your doctor is responsible for making some of the changes in the surgery they might see general feedback from patients.
Q Will the questions you ask me be medical or personal?
A We will only ask general questions about the practice, such as short questionnaires. We will initially collect anonymised data (age/ethnicity etc.) to ensure the group is representative of our patient population.
Q Who else will be able to access my contact details?
A Your contact details will be kept safely and securely and will only be used for this purpose and will not be shared with anyone else.
Q How often will you contact me?
A Not very often… [perhaps 3-4 times a year]
Q What is a patient group/patient participation group?
A This is a group of volunteer patients who are involved in making sure the surgery provides the services its patients need.
Q Will participation in this group affect my personal medical care?
A The purpose of the group is to help inform the practice as it strives to deliver high quality patient focused services. Your personal medical care and the relationship you have with your practitioner will not be directly affected by membership of this group. There is no link between membership of this group and your personal medical records.
Q What if I no longer wish to be on the contact list or I leave the surgery?
A We will ask you to let us know by email if you do not wish to receive further messages.
