The information below is taken from the Quality and Outcomes Framework (QOF) for 2007, which details the achievement of GP practices on various aspects of the care they provide. Below we show the QOF summary scores for asthma, coronary heart disease, diabetes and hypertension (high blood pressure), as well as an overall score for clinical achievement.
